While some providers may be required or choose to participate in TEAM, its principles can benefit all healthcare programs, regardless of official participation.
What is the CMS TEAM Model?
The CMS TEAM Model is a next-generation value-based care initiative that expands upon previous bundled payment programs. It aims to:

Drive cost savings while maintaining high-quality outcomes
Encourage data-driven decision-making
Promote collaboration among care teams
Reduce unnecessary hospital readmissions
